Darth Vader Flashback with 2 Left Hands!

I have had a Darth Vader Episode I Flashback Photo with two left hands in my collection for a while now. I have finally acquired a digital camera so that I could submit decent photos of it and here they are.

Has anyone else seen this error? (Other pics to follow)

That's a funny error, I can see why it happened though - 2 black pieces that are the same shape and size. This is the first I've seen though.

I have the same mistake by a Luke Flashback figure.
Is this common or allso a one in a million mistake??
